Position Summary:

Home Care is in high demand and we are looking for
experienced and caring LPNs to join our team at Home Health Care Incorporated!
Successful applicants will have a proven track record as an LPN, with home care
experience a huge benefit but not required. We encourage you to apply today!


Responsibilities:


  • Provide patient
    care according to client’s care plan and needs

  • Perform wound
    care treatments and assist clients with medication management

  • Teach the
    client and/or family members about care and support skills

  • Communicate
    regularly with RN Case Managers, Supervisors, and the Care Team


Qualifications:


  • Licensed as a
    Practical Nurse / LPN in the state of Minnesota

  • Driving is
    required; must have a reliable vehicle with current insurance and a valid
    Driver's License

  • Demonstrated
    ability to work autonomously

  • Prior Nursing
    experience in a TCU, Hospice, or Home Health setting

  • Experience with
    Electronic Medical Records (EMR) software is preferred
