Deliver skilled nursing care including medication administration, wound care, IV therapy, and catheter care
Conduct thorough assessments and develop individualized care plans
Monitor patient progress and respond to changes in condition with clinical accuracy
Educate patients and family members on care plans, medications, and health management
Document all care provided in compliance with regulatory and agency standards
Uphold the highest standards of patient care, safety, and professional ethics
Collaborate effectively with interdisciplinary teams and external providers
Active LPN License in the State of New York
Minimum 1–2 years of nursing experience (home care, pediatrics, or geriatrics preferred)
Strong clinical judgment and the ability to work independently
CPR/BLS certification (required)
Proficient in electronic health records (EHR) systems (preferred)
Excellent communication, organizational, and interpersonal skills
Reliable transportation and/or valid driver's license preferred
