Job Description

Licensed Practical Nurse (LPN) – Per Diem, Day Shift

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with established medical and nursing care plans. The LPN administers medications and treatments, monitors resident conditions, and collaborates closely with the Nurse Manager and Charge Nurse to ensure high-quality, compliant, and compassionate care.

Key Responsibilities

  • Administer and document all prescribed medications and treatments in accordance with physician orders, facility policies, and New York State Department of Health regulations

  • Monitor, assess, and evaluate residents’ conditions, including behaviors, symptoms, reactions, and overall progress

  • Communicate observations and clinical findings to the Nurse Manager and/or Charge Nurse in a timely and accurate manner

  • Perform treatments as ordered, following established clinical protocols and standards of care

  • Identify and discuss the need for medication or treatment changes with the Nurse Manager or Charge Nurse

  • Order prescribed medications, notify the pharmacy of discontinued medications, and verify accuracy of medications and dosages upon receipt

  • Maintain accurate, timely, and thorough clinical documentation

  • Work collaboratively with interdisciplinary team members to promote optimal resident outcomes and quality of care

Qualifications

  • Current and active New York State Licensed Practical Nurse (LPN) license

  • Strong knowledge of medication administration, nursing procedures, and regulatory requirements

  • Excellent communication, observation, and documentation skills

  • Ability to work effectively under the supervision of nursing leadership

Education

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
