University Home Care Inc is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Westland, MI. This role involves providing high-quality nursing care to our patients in their homes, ensuring their comfort and well-being while supporting their health care needs.

Responsibilities

• Provide direct nursing care to patients according to physician directives and care plans.
• Monitor patient health by taking vital signs and observing overall condition.
• Administer medications and injections as prescribed.
• Maintain accurate and detailed patient records and documentation.
• Collaborate with healthcare professionals and family members to develop and adjust care plans.
• Educate patients and families on treatment plans, care techniques, and health maintenance.
• Assist with basic patient care, including hygiene and mobility support as needed.
• Respond promptly to patient emergencies and communicate issues to supervisors.

Requirements

• Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
• Prior experience in home health care or a related nursing environment is a plus.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation to travel to patient homes within Westland, MI area.
• Basic computer skills for documentation and reporting.
