University Home Care Inc is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Westland, MI. As an LPN, you will provide essential nursing care to our patients, ensuring their health and comfort in a home care setting. Responsibilities

• Provide direct nursing care to patients as prescribed by physicians and in accordance with state regulations.
• Monitor patient health and record signs, symptoms, and changes in patient conditions.
• Administer medications and treatments safely and accurately.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Educate patients and their families on health management and care procedures.
• Maintain accurate documentation of all nursing care provided.
• Assist with any other reasonable duties as assigned to support patient care.

Requirements

• Current and valid Licensed Practical Nurse (LPN) license in the state of Michigan.
• Previous experience in home care or a similar healthcare environment preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-focused approach to care.
• Basic computer skills for documentation and communication.
