Licensed Practical Nurse LPN Full Time Home Health

We are looking for a compassionate Licensed Practical Nurse to join our growing Portland team. This full-time position serves patients throughout Portland and Cumberland County.

What You’ll Do

  • Provide skilled nursing care in patients’ homes under the direction of an RN and physician
  • Follow individualized plans of care
  • Monitor patients and report changes in condition
  • Administer medications and treatments within your scope of practice
  • Educate patients and caregivers
  • Complete timely and accurate clinical documentation
  • Promote patient safety and independence
  • Collaborate with nurses, therapists, physicians, and other members of the care team
  • Participate in team meetings and the on-call rotation

What You’ll Need

  • Current and unrestricted Licensed Practical Nurse license in Maine
  • Graduate of an accredited practical nursing program
  • At least one year of experience in an acute care or comparable clinical setting
  • Current CPR certification through the American Heart Association or American Red Cross
  • Strong communication, organization, and documentation skills
  • Compassionate and patient-focused approach to care
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Ability to travel between patient homes throughout Portland and Cumberland County
