Genesis Home Care
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
At Genesis Home Care, our Licensed Practical Nurses (LPNs) do more than provide care — they help patients heal comfortably and confidently in the place they call home. As an LPN with Genesis Home Care, you’ll provide skilled nursing care in patients’ homes under the direction of the attending physician and case manager. You’ll play a key role in improving patient outcomes, preventing re-hospitalizations, and helping patients regain independence safely at home.
Key Responsibilities:
Deliver skilled nursing care according to physician orders and care plans
Monitor patient conditions and communicate changes appropriately
Administer medications and treatments as prescribed
Educate patients and families on disease processes and preventative care
Complete accurate and timely clinical documentation
Follow agency policies, procedures, and Medicare/Federal guidelines
Current Licensed Practical Nurse (LPN) license in the state of employment
Minimum of one year of LPN experience in an acute care setting
Home health experience preferred
Valid driver’s license and active auto insurance
Ability to meet physical job requirements
