Licensed Practical Nurse (LPN) - Auburn Crest Primary & Palliative Care

About the Role

At Auburn Crest Primary & Palliative Care, we believe every patient deserves dignified, compassionate care especially during life's most difficult moments. As an LPN on our team, you'll provide hands-on nursing care to patients working alongside our team of Nurse Practitioners. This is meaningful work for a nurse who wants more than a routine, someone who brings both clinical skill and genuine human compassion to every visit.

What You'll Do

  • Conduct head-to-toe patient assessments and monitor vital signs, noting changes in physical or psychological condition
  • Administer medications and treatments with a focus on pain relief, nausea control, and wound care
  • Educate patients and families on care plans and disease progression, helping them make informed decisions with confidence
  • Collaborate with a care team to update individualized care plans and coordinate seamless transitions across care settings
  • Maintain accurate, compliant electronic health records in accordance with HIPAA and state regulations
  • Provide emotional support and stability to patients and families during end-of-life situations
  • Travel within the specified region to deliver in-home services

What We're Looking For

  • Active, unencumbered LPN license in the state of Idaho
  • Valid Driver's License and reliable transportation
  • Current CPR/BLS certification
  • 1-2 years of clinical experience, home healthcare experience preferred
  • Ability to work independently across varied home environments
