As a Home Health LPN, you’ll provide one-on-one skilled nursing care, build meaningful relationships with patients and families, and work alongside a supportive interdisciplinary team focused on positive patient outcomes.

What You’ll Do
Provide skilled nursing care in patients’ homes
Follow physician orders and care plans created by the Case Manager
Monitor patient conditions and report changes
Administer medications and treatments as prescribed
Educate patients and families using teach-back methods
Help reduce hospital readmissions through patient education and preventative care
Complete clinical documentation in accordance with agency and Medicare guidelines
Communicate schedule or availability changes with the appropriate team members
Qualifications
Current Licensed Practical Nurse (LPN) license
Minimum of 1 year of LPN experience in an acute care setting
Home health experience preferred
Valid driver’s license and active auto insurance
Strong communication, time management, and documentation skills
Compassionate, dependable, and patient-focused
