Per Diem Licensed Practical Nurses (LPNs) – Home Care
Service Areas: Westchester & Northern Westchester County
Setting: In-Home Adult Care

At Affirmed Home Care, we are redefining excellence in home health by uniting clinical expertise with genuine compassion. We’re seeking per diem Licensed Practical Nurses (LPNs) who are committed to providing high-quality, individualized care in the comfort of our clients’ homes.
We currently have 12-hour day and overnight shifts, as well as nursing visit opportunities, available throughout Westchester County.

What We’re Looking For
  • Minimum 1 year of recent LPN experience in an acute care setting (home care experience preferred)
  • Active NYS LPN license and current BLS certification
  • Eligible to work in the U.S.
  • Physical exam (within 1 year)
  • PPD or QuantiFERON (within 1 year) or chest X-ray (within 5 years)
  • MMR immunizations (within 10 years)
  • Valid driver’s license (preferred)
