Responsibilities:
- Participates in the delivery of care as set by the care plan created by the RN Case Manager and the interdisciplinary team.
- Assists with planning and implementing ongoing audit reimbursement program in accordance with federal, state, and Hospice standards, under the supervision of a registered nurse.
- Willingness to be flexible and agile as a field LPN.
- Reports findings and concerns to the RN Case Manager.
- Audits one complete chart for each field nurse in the QAPI team monthly.
- Possesses adequate knowledge of Medicare/Medicaid hospice benefit and insurance standards to ensure reimbursement for all services rendered and the regulatory standard of care is provided.
- Makes visits as scheduled
- Maintains patient rights and dignity. Assures the care of the dying patient, optimizing his/her comfort and dignity.

QualificationsRequirements:
- Successful completion of LPN or LVN training program

Experience: 1 year related
- Previous hospice or home health experience a plus
- Current LPN license; driver’s license and current automobile liability insurance
