Qualifications

• Minimum 1 year of current LPN experience in an acute care setting (home care experience preferred)

• Authorization to work in the United States

• Valid NYS LPN license

• Valid BLS certification

• Valid driver’s license (preferred)

• Physical exam within the past year

• QuantiFERON within the past year or chest X-ray within the past five years

• MMR immunizations within the past ten years
