Sentara Home Care is seeking to hire an LPN Intake Coordinator.
Position Status: Flexi, Days, Weekend hours as needed
Position Location: Remote with occasional onsite meetings and trainings in Chesapeake, VA. Eligible candidate must live no more than one hour away from Chesapeake, VA.
Hourly Range: $17.31 - $28.80

Evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients. Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues.
Education
Highschool Graduate or Equivalent
Certification/Licensure
LPN- Active in Virginia
Experience
Minimum 1 year of experience with clinical and customer service in a healthcare environment
Strong care coordination experience is highly preferred
Home Health experience is highly preferred
