As an LPN Visit Nurse with Home Health Care, Inc. , that's what your workday is built around. You'll travel to patients throughout your local area and provide scheduled skilled nursing visits one patient at a time. Assess what you're seeing. Provide the treatment. Answer the questions. Educate the family. Make sure they understand what happens next. Then move on to your next patient. Sometimes better nursing starts with simply having the opportunity to focus. You're Not Just There to Complete a Task A dressing change might be why the visit was scheduled. But when you walk through the door, you may notice something else. The patient doesn't understand their new medication. A caregiver has questions about the treatment plan. The wound doesn't look like it did last week. The patient mentions a symptom they hadn't reported. Something about their condition simply doesn't look right. Home health gives you the opportunity to observe, educate, communicate, and advocate while providing the skilled nursing care the patient needs. Because great nursing isn't just about completing the ordered treatment. It's about paying attention to the person receiving it. Work Close to Where You Live Your territory shouldn't automatically mean the entire Twin Cities. Patient visits are scheduled within approximately a 35-mile radius of your home , helping create a service area centered around where you live. You'll spend your day traveling between patients in your local area while receiving mileage reimbursement between patient visits. What Your Visits Could Include Skilled nursing observations and assessments Wound care and dressing changes Medication management and education Injections and ordered treatments Chronic disease monitoring Patient and caregiver education Reinforcing physician orders and plans of care Monitoring for changes in patient condition Communicating concerns to RN Case Managers and physicians Collaborating with therapists and other interdisciplinary clinicians Accurate and timely clinical documentation Every visit is different, but the goal remains the same: Help the patient remain healthy, informed, independent, and safely at home.
