Department: Home Care
Shift/schedule: Full Time (40 hrs/wk), Day Shift

GENERAL SUMMARY

Works under the supervision of the Nurse Manager. The Staff LPN's primary job function is to make home visits to render nursing care in accordance with the physician's plan of care and assignment. Completes clinical records on each visit. Accountable to the scope of practice outlined by the Ohio Board of Nursing under the direction of the registered nurse. Assists with QAPI programs; participates in weekend and holiday rotations. Performs other duties as assigned.

QUALIFICATIONS

Education:

• High School Diploma or successful completion of an equivalent High School Exam required

• Graduate from an accredited school of practical nursing required

Licensure:

• Licensed to practice as an LPN in Ohio required

• Licensed to practice as an RN in Kentucky to be obtained after hire required

• BLS within 90 days of hire required

• BMVL required (Must maintain eligibility to drive as determined by SOMC insurance carriers)

Experience:

• Prior home care experience preferred

• Approximately one year prior medical/surgical experience required.

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.

1. Makes home visits to render professional nursing care in accordance with physicians plan of care and assignment.

2. Completes clinical records on each visit. Maintains confidentiality of patient information.

3. Communicates changes in patient status to patients, family, physicians and other health care team members.

4. Teaches the patient, family or significant others health restoration and maintenance procedures. Observes return demonstrations of skills taught to patients and caregivers.

5. Observes patient response to teaching, treatment and medications. Documents patient response in the clinical record.

6. Always reports adverse reactions and abnormal signs and symptoms to the physician in a timely manner.

7. Makes recommendations to RN Case Managers for alterations in the plan of care.

8. Participates in staff meetings.

9. Pre-cleans, decontaminates and transports contaminated instruments according to policy

10. Performs intermediate-level disinfection and/ or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU’s
