This Opportunity:

The primary responsibilities of the Licensed Practical Nurse, Crisis are to provide direct and indirect services, under the supervision of a Registered Nurse, to individuals in need of crisis assessment, injection clinic, or outpatient services.

What You'll Do:

  • Schedule individuals for crisis and assessment services and triage individuals who present with emergent medical and/or psychiatric problems.

  • Perform clinical intakes with individuals requesting services, coordinate with screening/referral and individuals to ensure continuity of care.

  • Complete documentation required to ensure that the medical record for crisis and assessment services meets state and accreditation standards, coordinating with STR, psychiatrists/nurse practitioners, providers, and crisis services staff.

  • Schedule individuals for the Injection Clinic as well as doing injections of psychotropic medication, educate and support individuals about medication.

  • Complete documentation required to ensure that the medical record for crisis and assessment services meets state and accreditation standards, engage with drug company representatives who supply samples or coupons.

  • Deal effectively with a variety of people, which includes individuals requesting services, staff, providers, and the public.

  • Input electronic medical record documentation, including checking for Medicaid, IPRS, and Medicare status of individual receiving services.

  • Mail correspondence to individuals receiving services and the provider community.

  • Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.

  • Demonstrate knowledge of and comply with all agency policies and procedures, as well as service definitions related to specific program areas

  • Demonstrate knowledge of emergency procedures and assists in crisis situations.

  • Complete all other relevant responsibilities as assigned by the supervisor.

  • Driving and travel may be required.

Education We're Looking For:

Associates: Nursing, High School Diploma (Required)

Certifications We're Looking For:

Drivers License (Valid) - USA, Licensed Practical Nurse (LPN) - State Board of Nursing

Experience We're Looking For:

Experience working with adults with a mental health diagnosis and/or substance use disorder | 1 Year | Not Required

Schedule:

This is a PRN position. Shift coverage on an as-needed basis.
