We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. POOL/ per diem shifts available must be able to work 4 shifts a month including one weekend.

Key Responsibilities:

• Administer prescribed medications and treatments accurately and safely
• Monitor residents’ health status and report any changes to the nursing team
• Assist with activities of daily living, including bathing, dressing, and mobility
• Maintain detailed and accurate patient records in compliance with healthcare regulations
• Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement care plans
• Ensure a clean, safe, and comfortable environment for residents
• Follow all policies and procedures related to patient care and safety

Requirements

Skills and Qualifications:

• Valid and current LPN license in the state of employment
• Proven experience in a skilled nursing or long-term care setting preferred
• Strong clinical skills and knowledge of nursing practices
• Excellent communication and interpersonal skills
• Ability to work effectively as part of a multidisciplinary team
• Compassionate, patient-focused, and attentive to residents’ needs
• Ability to handle sensitive situations with professionalism and discretion
