Job Description

We are seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our Home Health Team in Naples. This role provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Is responsible and accountable for making decisions that are based on the individual's nursing experience and educational preparation.

What You'll Do

  • Provide skilled nursing care in accordance with physician orders, nursing standards, and the established plan of care.
  • Collaborate with Registered Nurses to implement, evaluate, and update patient care plans.
  • Observe, assess, and report changes in patients' physical, emotional, and mental health status.
  • Assist with treatments, procedures, medication administration, and other nursing interventions as permitted by state regulations and agency policies.
  • Educate patients and caregivers on disease management, treatment plans, medications, and self-care techniques to promote independence and wellness.
  • Document patient assessments, treatments, and progress accurately in the electronic medical record (EMR) within required timeframes.
  • Provide compassionate support that promotes patient comfort, dignity, safety, and quality of life.
  • Communicate effectively with patients, families, physicians, and interdisciplinary team members to ensure coordinated care.
  • Participate in team meetings, case conferences, and ongoing professional development opportunities.
  • Maintain compliance with agency policies, clinical standards, infection prevention protocols, and state regulatory requirements.
  • For qualified advanced LPNs, perform approved IV therapies and specialized procedures based on documented competencies and applicable regulations.

What We're Looking For

Required Qualifications:

  • Graduate of an accredited School of Practical Nursing.
  • Current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
  • Current CPR/BLS certification or ability to obtain within required timeframe.
  • Knowledge of nursing principles, patient care practices, medication administration, and infection prevention standards.
  • Ability to accurately assess, observe, and report changes in patient condition.
  • Experience documenting patient care in an Electronic Medical Record (EMR) system.
  • Strong communication and interpersonal skills with the ability to build trusting relationships with patients, families, and healthcare professionals.
  • Ability to work independently while collaborating effectively with Registered Nurses and interdisciplinary care teams.
  • Commitment to providing compassionate, patient-centered care while maintaining patient dignity, safety, and confidentiality.
  • Ability to meet the physical requirements of the position, including standing, walking, bending, lifting, and assisting patients as needed.

Preferred Qualifications:

  • Minimum of one (1) year of nursing experience, preferably in home health, hospice, long-term care, or a community-based healthcare setting.
  • Experience developing and implementing patient education plans.
  • Familiarity with Medicare, home health regulations, and clinical documentation requirements.
  • IV certification or documented competencies to perform advanced LPN duties in accordance with state regulations and organizational policies.
  • Bilingual skills are a plus.

Compensation Range:

$24.20

to

$36.30

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
