Job Description

  • Directs and supervises Certified Nursing Assistants.
  • Provides direct care to residents, as a necessary; assists other staff members with care as needed. May administer medications, treatments and tube feedings.
  • Maintains and monitors the physical environment of the unit to ensure the safety and well being of residents and staff. Ensures that utility rooms are in order and emergency supplies are present and ready for use.
  • Assists the Attending Physician on rounds, as necessary.
  • Reports changes in residents’ conditions to Physician and develops an appropriate plan of care.
  • Transcribes Physician’s orders correctly and completely as needed.
  • Carries out Physician’s orders according to established policies. Ensure that all medications and treatments are administered in accordance with written Physician’s orders.
  • Oversees preparation of co-workers assignments and monitors same for satisfactory completion.
  • Initiates and updates CCP.
  • Makes frequent rounds on all residents to ascertain the resident’s condition, quality of care, environment problems, complaints and application of plan of care.
  • Observes and assists in the dining room during resident meal times and ensures that all staff are accurate.
  • Receives and gives report at change of shift and conveys resident information to the staff when giving assignments.
  • Reports all unusual occurrences/emergencies to RN, Nurse Manager or Supervisor.
  • Handles all emergencies which may occur.
  • Collaborates with other department staff to meet the total needs of residents.
  • Completes documentation assigned for compliance with all New York State Safety and Infection Control Codes.
  • Participates in the Performance Improvement Plan through completion of audits and developing/implementing corrective actions.
  • Establishes PAR levels for supplies and sees that supplies are obtained and updated as needed.
  • May initiate admission assessment on new residents, re-admits, hospital returns and initial care plan.
  • May gather information related to resident initial assessment and discharge planning.
  • Works well with interdisciplinary team, residents, and family members to ensure the highest quality of resident care.
  • Assists new nursing staff in orientation and unit teaching.
  • Demonstrates proficiency in use of the following equipment to include but not limited to: blood pressure cuff, feeding pumps, ear thermometer, suction machine, glucometer, weight scales, fire extinguisher, call bell system, oxygen concentrator/tank, and hoyer lift.
  • Follows all established Policies & Procedures of The Wartburg.
  • Performs all other duties deemed necessary by the DON/designee.

Qualifications:

Experience

  • Preferably 2 years experience in Long-Term care.

Education

  • Graduate of an accredited school of nursing
  • Current NYS LPN licensure
  • Current CPR certification

Pay Range


$39.5532 per hour
