The Licensed Practical Nurse (LPN) partners with the Director of Nursing to oversee and support the medical care of individuals served within our residential programs. This position promotes a collaborative "One Team" approach by working closely with medical, clinical, and operational teams to ensure the highest quality of care.
The LPN will travel to assigned residential homes, provide medication management, support medical treatment needs, educate staff and individuals served, and assist Direct Service Providers with healthcare-related concerns and best practices.

Primary Responsibilities
• Maintain compliance with all company policies, procedures, and regulatory requirements.
• Conduct daily follow-up and in-person monitoring of resident health concerns at assigned locations.
• Administer medications, injections, and vaccinations as prescribed by healthcare providers.
• Complete nursing assessments, diabetic foot assessments, AIMS assessments, and ensure Health Care Appraisals (HCAs) are completed timely.
• Coordinate continuity of care through follow-up on physician orders, medication changes, hospital admissions and discharges, specialty appointments, and treatment plans.
• Monitor medication effectiveness, potential side effects, and drug interactions.
• Review medication administration records, medication counts, and medication orders according to healthcare requirements.
• Assist with scheduling and coordination of psychiatric, medical, dental, and vision appointments.
• Participate as an active member of the interdisciplinary care team and attend team meetings as assigned.
• Collaborate with team members to address barriers to treatment, including medication or treatment refusal.
• Provide Direct Medication Administration (DMA) and medical skills training when applicable.
• Participate in the medical on-call rotation.
• Assist with the development and revision of medical policies and procedures.
• Support controlled substance destruction processes in accordance with DEA guidelines.
• Triage medical concerns and provide clinical guidance as needed.
• Audit electronic health record (EHR) and electronic medication administration record (eMAR) systems.
• Educate staff and individuals served on infection prevention, medication management, physical health needs, and medical equipment use.
• Serve as a healthcare liaison for families, guardians, and external providers.
• Provide data and support for Infection Control Plan initiatives and reporting.
• Participate in annual influenza prevention campaigns and reporting efforts.
• Perform other duties as assigned.

Qualifications - Required Education and Experience
• Current and valid Licensed Practical Nurse (LPN) license in the applicable state.
• Minimum of two (2) years of nursing experience.
• Ability to maintain all required certifications, licenses, and mandatory training.
• Successful completion of a criminal background check.
• Valid driver's license with reliable transportation.
• Authorization to work within Beacon programs as approved by applicable state and federal agencies.
• Proficiency in speaking, reading, and writing English.
• Ability to utilize office equipment and electronic healthcare systems.

• Skilled in conflict resolution and de-escalation techniques.
• Ability to provide direct nursing care and respond to changing medical needs.

• Excellent computer proficiency, including experience with electronic health records.
• Strong verbal and written communication skills.
• Ability to work independently while maintaining a team-oriented approach.
