Envision Unlimited is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to support our community living programs across Aurora, Kankakee, and Tinley Park, IL In this dynamic, hands-on role, you’ll play a critical part in promoting the health, safety, and well-being of the individuals we serve.
Working under the supervision of a Registered Nurse, you’ll conduct onsite assessments, administer medications, monitor treatment plans, support hospitalizations, and collaborate with interdisciplinary teams to ensure high-quality care. The Licensed Practical Nurse (LPN) at Envision Unlimited plays a vital role in assessing and addressing the medical, health, and safety needs of clients within assigned community living program(s). Under the supervision of a licensed Registered Nurse, the LPN conducts client assessments, documents medical needs and interventions, monitors medication regimens, and collaborates with internal and external partners to ensure client well-being.
