The PT Builds the Plan. You Help Turn It Into Progress. An evaluation identifies the problem. A plan of care establishes the goals. But recovery doesn't happen because a goal was written down. It happens through everything that comes next. The exercises. The repetition. The encouragement. The adjustments. The education. The days when the patient doesn't think they're improving—but you can see that they are. As a Physical Therapist Assistant with Home Health Care, Inc. , you'll be the clinician working alongside patients throughout that process. Visit after visit, you're helping turn the plan into progress. Be the Person Who Keeps Them Going Recovery isn't always linear. Some patients are motivated from day one. Others are scared to fall again. Some are frustrated by how much strength they've lost. Some don't believe they'll ever get back to where they were. That's where a great PTA makes a difference. You aren't simply counting repetitions. You're teaching. Correcting. Encouraging. Progressing. Watching. Listening. And helping your patient recognize that the things they couldn't do two weeks ago are becoming possible again. Home health gives you the opportunity to build that relationship one-on-one. No crowded therapy gym. No bouncing between multiple patients at once. Just you, your patient, and the next goal you're working toward together. Your Territory: Western Metro & Buffalo Area You'll provide home-based Physical Therapy throughout communities west and northwest of the Twin Cities, including: Cokato, Corcoran, Delano, Greenfield, Hamel, Howard Lake, Independence, Long Lake, Loretto, Maple Plain, Medina, Montrose, Mound, Rockford, Waverly, Watertown, Hanover, Annandale, Kimball, Maple Lake, South Haven, Buffalo, and Cold Spring. Patient scheduling is coordinated geographically whenever possible to help cluster visits and create a more efficient field schedule. What Progress Might Look Like For one patient, success is walking independently from the bedroom to the kitchen. For another, it's getting into the shower safely. It might be: Standing from a chair without assistance Walking with greater confidence Improving balance after a fall Regaining strength following surgery Navigating stairs again Transitioning to a less restrictive assistive device Increasing endurance after hospitalization Safely transferring in and out of bed Getting outside for the first time in weeks Becoming less dependent on a spouse or caregiver These aren't exercises for the sake of exercise. They're steps toward getting someone's independence back. What You'll Do Working from the plan of care established by the supervising Physical Therapist, you'll: Provide one-on-one Physical Therapy treatments in patients' homes Carry out individualized treatment plans Provide therapeutic exercise and strengthening Perform gait and mobility training Work on balance, coordination, endurance, and transfers Reinforce home exercise programs Educate patients and caregivers on safe mobility techniques Help patients build confidence with functional activities Monitor patient response and tolerance to treatment Identify and communicate changes in patient condition Report progress to the supervising Physical Therapist Collaborate with nurses, OT, SLP, physicians, and interdisciplinary clinicians Complete accurate and timely clinical documentation You'll See the Progress Others Might Miss The first time they stand without pushing off with both hands. Compensation That Rewards Your Work This is a per-visit position with annualized earning potential of $70,000–$102,000 per year. For a motivated PTA who can independently manage an efficient field schedule, the per-visit model provides an opportunity to build strong earning potential while maintaining the autonomy that makes home health attractive. You'll also receive mileage reimbursement between patient visits. Clinical skill matters. But so does your ability to connect with people. We're looking for someone who can walk into a patient's home and make them feel confident that progress is possible. Someone who knows when to encourage. When to challenge. When to slow down. When something needs to be communicated to the PT. And when a patient is capable of doing just a little more than they think they can. You'll need: Active Minnesota Physical Therapist Assistant (PTA) license Strong therapeutic exercise and functional mobility skills Excellent patient and caregiver communication Strong organization and time management Ability to independently manage a field-based schedule Confidence working one-on-one in patients' homes Strong collaboration with supervising Physical Therapists Valid driver's license and reliable transportation Ability to travel throughout the assigned Western Metro & Buffalo-area territory Home health experience is welcomed, but not required . PTAs coming from outpatient therapy, skilled nursing, TCU, inpatient rehabilitation, orthopedics, assisted living, and other rehabilitation settings are encouraged to apply.
