What You'll Do

• 
  Carry out individualized treatment plans developed by the supervising Physical Therapist.
• 
  Provide therapeutic exercises, gait training, transfer training, balance activities, and functional mobility interventions.
• 
  Reinforce home exercise programs and educate patients and caregivers.
• 
  Monitor patient progress and communicate updates with the supervising PT.
• 
  Collaborate with physicians, nurses, and the interdisciplinary care team.
• 
  Complete accurate and timely clinical documentation.

What We're Looking For

• 
  Active Minnesota
  Licensed Physical Therapist Assistant (PTA)
  license.
• 
  Passion for helping patients regain independence.
• 
  Strong communication, organization, and patient education skills.
• 
  Home health experience is preferred but not required.
• 
  Valid driver's license and reliable transportation.
