Are you a compassionate, detail-oriented Licensed Dietitian (RD)looking to make a difference in the lives of residents in a long-term care setting? We are seeking an experienced and motivated professional to join our interdisciplinary healthcare team. This is a hybrid position, with the majority of work performed on-siteto ensure exceptional resident care and collaboration with our clinical and dietary teams.

The Licensed Dietitian is responsible for providing comprehensive nutrition assessment, planning, monitoring, and education for residents while ensuring compliance with federal, state, and facility regulations. This role partners closely with nursing, physicians, dietary services, and other healthcare professionals to optimize resident outcomes.

Key Responsibilities

  • Complete nutritional initial, quarterly, annual, and significant change assessments according to federal and state regulations.
  • Conduct monthly nutrition reviews for high-risk residents, including those experiencing:
    • Significant weight loss or gain
    • Pressure injuries
    • Hemodialysis
    • Enteral (tube) feeding
  • Develop, implement, and monitor individualized nutrition care plans.
  • Provide nutrition counseling and education to residents, families, and staff as appropriate.
  • Coordinate nutrition care services with the Dietary Supervisor and interdisciplinary care team.
  • Collaborate with physicians, nurse practitioners, dialysis dietitians, nursing staff, and other healthcare professionals regarding resident nutritional needs.
  • Monitor food service operations to ensure compliance with nutritional, sanitation, safety, quality, and regulatory standards in tandem with the dietary manager.
  • Evaluate food temperatures, portion control, preparation methods, presentation, and quality.
  • Inspect diet trays for compliance with physician diet orders prior to meal delivery.
  • Participate in daily interdisciplinary meetings and contribute to resident care planning.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Stay current with federal, state, and professional practice standards.

Required Qualifications

  • Licensed Dietitian (LD) credential with current state licensure or eligibility, as required.
  • Minimum of two (2) years of hands-on experience managing Total Parenteral Nutrition (TPN) and Clinimix therapy is required.
  • Minimum of two (2) years of clinical nutrition experience, preferably in long-term care, skilled nursing, rehabilitation, or healthcare.
  • Thorough understanding of federal and state regulations related to long-term care nutrition services.
  • Strong assessment, documentation, and clinical decision-making skills.
  • Excellent communication and collaboration skills.
  • Ability to manage multiple priorities while maintaining a high standard of resident care.

Preferred Qualifications

  • Experience in skilled nursing or long-term care.
  • Experience with PointClickCare.
  • Knowledge of renal nutrition, wound care nutrition, and enteral nutrition support.
