Atrium Health Wake Forest Baptist Pediatric Clinic at Westbrook is seeking a compassionate and collaborative Licensed Clinical Social Worker (LCSW) to join our interdisciplinary care team. This role is ideal for a social worker who brings a true team‑oriented, patient‑centered mindset, a strong appreciation for community resources, and creativity in problem‑solving to support pediatric patients and their families. Conduct comprehensive psychosocial assessments to determine patient risk levels and needs
Participate in the development and implementation of individualized plans of care
Plan, arrange, implement, and evaluate psychosocial programs for patients and families
Develop and deliver education and counseling programs for individuals and groups
Serve as a liaison between patients, families, community organizations, and healthcare providers
Provide short‑term counseling and crisis intervention as needed
Identify and address psychosocial disorders, interpreting patient needs for professional and lay audiences
Support community and social programs in alignment with federal, state, and local grant or contract requirements
Prepare quarterly and ad‑hoc reports as required
Collaborate with clinical, health education, and community health staff to ensure coordinated care
Provide appropriate referrals and follow‑up for needed services
Utilize assessment tools, measurement scales, and objective criteria to evaluate patient needs and outcomes
Deliver in‑service trainings and presentations for staff as needed
Conduct community outreach and provide health‑related educational presentations
Perform additional responsibilities as assigned
Bachelor’s degree in Social Work, Counseling, Psychology, or a related field
Experience working with Medicare, Medicaid, and dual‑eligible populations
Licensed as a Clinical Social Worker (LCSW)
Master’s degree in Social Work or related field
Experience in an ambulatory health center
Bilingual in English and Spanish
Previous experience working in multicultural settings
Willingness to enroll in language and multicultural coursework if needed
Professional knowledge of breastfeeding processes and antepartum, intrapartum, and postpartum psychosocial care
Understanding of the needs of premature infants and pediatric patients from newborn through toddler years
Strong written and verbal communication skills, including solid English grammar and documentation ability
Excellent organizational skills and familiarity with healthcare policies, procedures, and protocols
Proficiency with computer systems and clinical applications
Knowledge of common safety hazards and universal precautions to ensure a safe work environment
