Atrium Health Wake Forest Baptist Pediatric Clinic at Westbrook is seeking a compassionate and collaborative Licensed Clinical Social Worker (LCSW) to join our interdisciplinary care team. This role is ideal for a social worker who brings a true team‑oriented, patient‑centered mindset, a strong appreciation for community resources, and creativity in problem‑solving to support pediatric patients and their families. Essential Functions

  • Conduct comprehensive psychosocial assessments to determine patient risk levels and needs

  • Participate in the development and implementation of individualized plans of care

  • Plan, arrange, implement, and evaluate psychosocial programs for patients and families

  • Develop and deliver education and counseling programs for individuals and groups

  • Serve as a liaison between patients, families, community organizations, and healthcare providers

  • Provide short‑term counseling and crisis intervention as needed

  • Identify and address psychosocial disorders, interpreting patient needs for professional and lay audiences

  • Support community and social programs in alignment with federal, state, and local grant or contract requirements

  • Prepare quarterly and ad‑hoc reports as required

  • Collaborate with clinical, health education, and community health staff to ensure coordinated care

  • Provide appropriate referrals and follow‑up for needed services

  • Utilize assessment tools, measurement scales, and objective criteria to evaluate patient needs and outcomes

  • Deliver in‑service trainings and presentations for staff as needed

  • Conduct community outreach and provide health‑related educational presentations

  • Perform additional responsibilities as assigned

Education, Experience, Licensure & Certification

  • Required:

    • Bachelor’s degree in Social Work, Counseling, Psychology, or a related field

    • Experience working with Medicare, Medicaid, and dual‑eligible populations

    • Licensed as a Clinical Social Worker (LCSW)

  • Preferred:

    • Master’s degree in Social Work or related field

    • Experience in an ambulatory health center

    • Bilingual in English and Spanish

    • Previous experience working in multicultural settings

    • Willingness to enroll in language and multicultural coursework if needed

Skills & Qualifications

  • Professional knowledge of breastfeeding processes and antepartum, intrapartum, and postpartum psychosocial care

  • Understanding of the needs of premature infants and pediatric patients from newborn through toddler years

  • Strong written and verbal communication skills, including solid English grammar and documentation ability

  • Excellent organizational skills and familiarity with healthcare policies, procedures, and protocols

  • Proficiency with computer systems and clinical applications

  • Knowledge of common safety hazards and universal precautions to ensure a safe work environment
