Best Home Health Providers, Inc. is a family-owned home health agency serving patients across the Bay Area with in-home skilled care and recovery support after discharge from medical facilities. Our work is centered on helping patients and families navigate care transitions with compassion, clarity, and clinical excellence.

As a Licensed Clinical Social Worker, you will play an important role in supporting patients through complex health and life circumstances. This position offers the opportunity to contribute meaningful clinical insight, strengthen care continuity, and help patients and families access the support they need during recovery.

Responsibilities

• Provide psychosocial assessments and develop individualized care and support plans.
• Offer counseling and crisis intervention for patients and family members.
• Coordinate with interdisciplinary teams to ensure continuity of care.
• Connect clients with community resources, benefits, and follow-up services.
• Maintain timely, accurate clinical documentation and compliance with regulations.

Requirements

Required Qualifications

• Active LCSW license in good standing with the State Board of California.
• Master of Social Work degree from an accredited program.
• 1 to 2 years of experience providing clinical counseling and crisis intervention in healthcare settings.
• Strong knowledge of community resources, care coordination, and discharge planning.
• Experience collaborating effectively with interdisciplinary teams.
• Experience maintaining timely and accurate clinical documentation.

Preferred Qualifications

• Experience with Devero.
• Background in home health.
• Experience supporting older adults and patients following discharge.
