FLSA Classification: PRN, Non-exempt, Hourly $30.42-$45.75

We’re looking for a charismatic individual to bring valuable experience and skills as a Licensed Clinical Social Worker (LCSW). The LCSW provides clinical social work services to patients and their families. Counsels and provides intervention for patients, including assessment, treatment, and care coordination as part of a multidisciplinary team.

Essential Functions:

  • Provide therapeutic services to patients and their families.
  • Work in collaboration with multidisciplinary team members including external partners.
  • Completes required documentation and paperwork including assessments, treatment planning and implementation in timely manner.
  • Develop and maintain social services, draft supporting policies, procedures and guidelines for the program. Develop and maintain a knowledge of referral resources throughout the service area.
  • Act as a resource person for other members of the healthcare team, provide in-service education and consultation of social and emotional factors related to health problems.
  • Maintain knowledge of the filled including Medicare, Medicaid and other third party reimbursement sources.
  • Participate in planning and implementation of quality improvement activities.
  • Adhere to department and hospital policies.

Required Skills and Abilities

  • Working knowledge of clinic policies and procedures
  • Knowledge and experience working with co-occurring populations, chronic health, mental health, or substance abuse.
  • Strong clerical/administrative skills including computer skills.
  • Ability to make routine decisions and determine proper action based on experience with the limits of policy/procedures.
  • Demonstrated oral/written skills in English.
  • Ability to work effectively with people of diverse cultures, ages, and economic backgrounds in a competent and sensitive manner.

Education & Experience:

  • 1 – 2 years’ hospital or medical clinic based experience preferred.

Licensure/Certification Required:

  • Master level degree from an accredited college/university in social work or related field.
  • LCSW in good standing in State of Colorado

About Pioneers Medical Center

Pioneers' history began in 1947 when Freeman Fairfield, a previous resident of this community, approached the County Commissioners about establishing a hospital. Over the next several years, Mr. Fairfield worked with the Commissioners and a group of local citizens to build a hospital for the Meeker community. Opening its doors in 1950, Pioneers began offering acute health care and surgical services to the community, licensed through the State of Colorado Department of Health and Human Services. We are Medicare and Medicaid-certified, accept private insurance, and, of course, self-pay. Financial arrangements may be set up through the Business Office.

In 1964, the Walbridge Memorial Convalescent Wing opened its doors for long-term care, made possible through the generosity of the wills of Carl and Adelaide Walbridge. Attached to the north side of the hospital, the Wing, with its 33 beds, brings skilled and intermediate nursing to residents who are unable to care for themselves. Through the Wing's rehabilitation services, exercise, and activity groups, each resident's care is individualized to meet his or her physical, emotional, nutritional, and spiritual needs. The Wing is licensed by the State of Colorado Department of Health and Human Services, and accepts Medicare, Medicaid, and private-pay residents.

First-line health care services are delivered through Meeker Family Health Center. The MFHC originated in 1990 as a rural health clinic, and its physicians, nurse practitioners, and staff continue to strive to meet our community's and visitors' healthcare needs.
