Full-Time | Day Shift | $25–$30 per hour

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Wellness team. The LPN will provide hands-on resident care, administer medications, maintain accurate documentation, and work closely with the Registered Nurse and care team to support residents’ health, safety, and well-being.

Responsibilities

• Provide resident care in accordance with individualized service plans.
• Assist residents with activities of daily living as needed.
• Administer prescribed medications and accurately document all services provided.
• Monitor residents for changes in health, behavior, or functional status and promptly report concerns to the Registered Nurse.
• Assist with developing and updating resident service plans and care documentation.
• Respond to resident emergencies, provide first aid, and arrange appropriate medical follow-up.
• Communicate with physicians and other healthcare providers as directed.
• Provide residents with compassionate one-on-one support and reassurance.
• Help maintain a safe, comfortable, and respectful environment for residents.
• Support the Registered Nurse with staff training, assignments, and other clinical responsibilities as needed.
• Follow all community policies, procedures, and applicable healthcare regulations.
• Promptly report resident incidents and work-related injuries.

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in the State of New Jersey.
• Successful completion of an approved practical nursing program and the required licensing examination.
• Strong communication, organizational, and documentation skills.
• Compassionate, dependable, and committed to providing quality senior care.
• Prior experience in assisted living, senior living, or long-term care is preferred.

Shift

• 7:00am-3:00pm
• 11:00pm-7:00am
• Weekends
