The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Pay Range:

$72,779.20 - $95,076.80

This role involves home visits to participants of the LIFE program.

ESSENTIAL FUNCTIONS

  • Evaluates participant based on pre-determined PACE program criteria to determine Level of Care (LOC) for both new enrollees and current participants

  • Effectively communicates assessment findings and participants needs with the IDT team.

  • Effectively communicates the PACE model to potential participants and families.

  • Utilizes knowledge to understand the physical, social, psychological and emotional needs of participants.

  • Inputs assessments in database accurately and timely based on regulations and policies. Ensures compliance to coding parameters, completion of all required fields, accuracy of the assessment coding, overall congruency of the coding and compliance with the assessment narrative guidelines.

  • Works independently in scheduling of and completing required tasks in the homecare setting.

  • Completes annual LOC assessments accurately and within required timeframes.

  • Assists in developing care plans based off of information obtained during assessment.

  • Completes and meets all requirements to maintain certification annually.

MINIMUM REQUIREMENTS

  • Bachelor's Degree in healthcare related or behavioral science field or licensed clinical or licensed certified social worker, or licensed registered nurse.

  • One year’s experience working directly with the elderly or physically disabled in an institutional or community setting. Certified in NJ Choice assessment or must attain within three months of hire.
