This job is responsible for working in a collaborative manner with multi-disciplinary medical center in the continuum of care of these patients, their caregivers and/or families to achieve desired goals. The incumbent will be accountable for planning, recording and following a course of action to assist patients and improve access to recommended medical services.

EXPERIENCE AND EDUCATION
Required- Education
Associate's Degree
- Experience
5 years experience in a health care environment
Preferred- Education

JOB DUTIES - Acts as a liaison between the patient, their family and internal/external service providers.
- Develops and implements individualized patient navigation plan for patients seeking treatment by facilitation of appointment scheduling, assisting with referral process, providing client education and linking clients with appropriate support services.
- Creates and maintains comprehensive training programs to orient and develop patient navigators.
- Supervises and facilitates development of patient navigator staff.
- Ensures that navigator staff has strong background and competence in supporting specific disease processes, or areas to which they are assigned.
- Identifies patient's understanding of medical care/outcome and provides appropriate education and resources to address needs.
- Promotes teamwork and multidisciplinary care concept to patients, staff and faculty.
- Works in conjunction with individual departments and health system leadership to develop procedures and policies for use by patient navigators in service to specific patient population areas.
- Promotes the navigation program to the UT Southwestern Medical Center (UTSW) community by conducting presentations to staff and faculty.
- Identifies and maintains a database of appropriate resources for client support.
- Rounds on identified patients, accessing patient and family needs, formulating a plan to meet needs; and to facilitate coordination of care with UT Faculty and community physicians.
- Duties performed may include one or more of the following core functions:
(a) Directly interacting with or caring for patients;
(b) Directly interacting with or caring for human-subjects research participants;
(c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or
(d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
- Performs other duties as assigned.
