Job Description

Under the direct supervision of the Pharmacy Manager or designee, the Pharmacist will be responsible for preparing and dispensing medications as prescribed and ordered by a licensed physician and in compliance with applicable regulations and standards; for assisting in the operation of the Pharmacy; for providing clinical consultation and expertise where needed and requested, in support of client assessment and care processes; for participating in the development and conduct of clinical, staff, and continuing education programs and for collaborating with other staff and leaders in the development, management and operation of the Pharmacy. Pharmacist will be responsible for ensuring USP 797/800 compliance and assist the department with Investigation Drug Services initiatives. Provides technical supervision to support personnel in the performance of pharmacy duties.

ESSENTIAL FUNCTIONS

  • Provides expert information to physicians, patients, and others about medications and other pharmaceuticals with respect to such factors as the appropriateness of medications in relation to the patient's condition and medication history; the choice of medications including dosage form, quantity, methods of administration, and alternative therapy for medications having undesirable side effects; the pharmacological characteristics of medications including their effects on the human system and the condition being treated and their chemical interaction with other substances in the patient's system; and their physical characteristics such as compatibility with other medications, forms in which they are or can be manufactured, and expected shelf life.
  • Conducts or participates in the orientation and training of pharmacy students, pharmacy technician trainees, and other professional or technical staff in pharmaceutical principles, practices, operations, and standards.
  • Screens prescriptions and medication orders for completeness, proper authorization, dosage and quantity, therapeutic compatibility, medication interactions and allergies, and clarifies possible problems with appropriate medical staff. All orders shall be processed and ready for scheduled delivery rounds conducted approximately every hour. A Pharmacist must accurately and quickly prepare all orders.
  • Selects, compounds, and dispenses medications and other pharmaceutical products ordered by physicians, dentists, and other licensed medical practitioners, including the calculation and preparation of solutions for oral, external, intramuscular, and intravenous administration. Ensure USP 797 / 800 compliance and documentation of appropriate standards.
  • Supervises or performs the preparation of medication labels and the affixing of labels to the proper containers; ensures the inclusion of appropriate directions for the timing, methods, and precautions to be observed in taking the medication; personally instructs patients in the use of medications when indicated.
  • Provides technical supervision to all staff including volunteers in the manufacture and prepacking of pharmaceuticals; participates in the development of techniques for compounding, manufacturing, or prepackaging pharmaceuticals.
  • Develops, maintains, and supervises the operation of medication distribution systems including the maintenance of appropriate controls; notes and reports quality deficiencies of medication products; ensures that pharmaceuticals are properly ordered, stored, kept up to date, and protected from deterioration.
  • Provides technical supervision to nonprofessional and subprofessional employees in the performance of a variety of pharmacy duties.
  • Attends and actively participates in meetings, conferences, and seminars for the purpose of upgrading professional skills and sharing information on recent developments in the fields of pharmacology and pharmaceutics.
  • Prepares any IV solutions as needed, using proper aseptic preparation techniques. Prepares any IV chemotherapy as needed, using proper aseptic and cytotoxic preparation techniques.
  • Following completion of medication cassettes by technicians, the Pharmacist will inspect the cassette for accuracy against a current patient profile.
  • Actively participates in medication safety programs. Active participation means identifying opportunities to improve safety (reporting errors and adverse drug events) and implementing system-based changes as a result of those opportunities.
  • Mentors' pharmacy students in applicable educational experiential training programs.

QUALIFICATIONS

Education - Required

  • Doctorate Graduation from Accredited Pharmacy school/ college

Experience - Required

  • 3 years experience of hospital pharmacy practice experience

Experience - Preferred

  • 1 year oncology or infusion experience

Licenses and Certifications - Required

  • RP - Registered Pharmacist - State of Pennsylvania
