Lead Community Health Worker

A Lead Community Health Worker (Lead CHW) is a seasoned, frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served. Under general supervision, provides day-to-day guidance to a team of Community Health Workers, who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, the Lead CHW will serve as a coach and mentor to Community Health Workers and ensure compliance with program guidelines. Acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.

Pay Range: $20.02- $31.03 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
