Summary
The Lead Case Manager serves as a senior clinical support and operational lead within the case management team, providing oversight, guidance, and coordination of case management services while maintaining a caseload. This role ensures continuity of care, high-quality documentation, effective interdisciplinary collaboration, and adherence to regulatory and organizational standards. The Lead Case Manager acts as a clinical resource for case management staff, supports program flow from admission through discharge, and helps translate clinical vision into day-to-day execution.
Key Responsibilities
- Provide clinical leadership and day-to-day guidance to Case Managers, serving as a point of support and escalation.
- Maintain a caseload while ensuring timely, person-centered case management services for assigned clients.
- Oversee coordination of care including admissions, treatment planning, utilization review support, discharge planning, and aftercare coordination.
- Ensure case management documentation meets clinical, regulatory, and payer requirements, including timeliness and accuracy.
- Participate in multidisciplinary treatment team meetings and support alignment across nursing, therapy, psychiatry, and operations.
- Support onboarding, mentoring, and ongoing development of Case Management staff.
- Monitor caseload distribution, workflow efficiency, and barriers to care progression, escalating concerns as needed.
- Collaborate with leadership on process improvement initiatives related to care coordination, transitions of care, and outcomes.
- Serve as a liaison with external providers, families, referral sources, and community partners when clinically appropriate.
- Model professionalism, ethical practice, and Mainspring’s values in all client, staff, and stakeholder interactions.

QualificationsMinimum

Qualifications
Education / Credentials
- Bachelor's degree in social work, or related behavioral health field required
Experience
- Minimum of 3–5 years of experience in behavioral health or substance use disorder treatment
- Prior experience in case management, care coordination, or utilization review strongly preferred
- Previous leadership, lead, or supervisory experience preferred
Knowledge / Skills
- Strong understanding of levels of care, treatment planning, discharge planning, and continuum-of-care models
- Working knowledge of ASAM criteria, documentation standards, and regulatory requirements
- Excellent written and verbal communication skills, with the ability to navigate complex clinical and family dynamics
- Strong organizational skills with the ability to prioritize, problem-solve, and manage competing demands
- Ability to work independently while also functioning as a collaborative member of an interdisciplinary team
Working Conditions
- Combination of direct client interaction and non-direct administrative/clinical leadership duties
- Full-time schedule, primarily weekday business hours, with occasional flexibility based on program needs
- Office-based clinical environment with regular computer use, meetings, and client engagement
