Summary:
This position supports Saban’s Street Medicine and Mobile Clinic programs by delivering direct outreach and care coordination to individuals experiencing homelessness, severe mental illness, and substance use disorders. This role is responsible for coordinating whole-person care services for members who meet the population of focus criteria and requirements set forth by the Health Plans. The position requires a balance of fieldwork and administrative responsibilities, with approximately 70% of time spent in the field with the Street Medicine team, 10% on the Mobile Van, and 20% on administrative tasks such as documentation, tracking outcomes, and reporting to the health plans.
Duties and Responsibilities:
- Walking on-foot to serve individuals experiencing homelessness, including homeless encampments and community sites.
- Welcomes patients and helps set up services for patient to be seen at encampment by a Street Medical Provider.
- Participate in street-based outreach with the Street Medicine team to locate, engage, and screen members for the Enhanced Care Management program.
- Track, report, and monitor patient enrollment, follow-ups, and outcomes to ensure quality standards and          compliance.
- Coordinate with hospitals, shelters, housing agencies, and community-based organizations to avoid duplication of services.
- Oversee provision of ECM services and implementation of individualized Care Plans.
- Complete timely and accurate documentation in the Electronic Health Record (EHR).
- Conduct follow-ups with patients via phone, encampment visits, and alternative sites where patient can be found.
- Accompany members to office or medical visits as needed and in alignment with MCP guidelines.
- Conduct initial needs assessments and assist in developing individualized care plans.
- Coordinate access to services, including: Primary care, specialty care, mental health, and substance use treatment.
- Inform patients about other services offered by Saban Community Clinic, including services for family members.
- Participate in case conferencing and interdisciplinary team meetings.
- Track outcomes related to engagement, linkage, and service utilization.
- Connect members to housing resources, benefits, transportation, and supportive services.
- Serve as a liaison between clients, Street Medicine providers, social service agencies, and housing authorities.
- Maintain strong relationships with community partners and service providers to streamline referrals.
- Advocate for patients with healthcare professionals and community partners.
- Coordinate with hospital staff on discharge planning.
- Monitor treatment adherence, including medication management.
- Provide health promotion, health education, and self-management training.
- Use motivational interviewing, trauma-informed care, and harm-reduction approaches.
- Performs additional duties as assigned.
Position Requirements:
- Bachelor and/or master’s degree in social work, nursing, related fields or equivalent experience
- Minimum 3 years of care management or case management experience serving vulnerable or homeless populations.
- Prior experience with street-based outreach, mobile health, or community-based care strongly preferred.
- Strong understanding of social determinants of health, trauma-informed care, and harm reduction practices.
- Ability to build trust with individuals experiencing homelessness in non-traditional settings.
- Must be able to walk and stand for extended periods during outreach in community settings.
- Valid California driver’s license and reliable transportation required.
- Flexibility to work in-office, in the field, and on the mobile clinic as program needs dictate.
- Self-starter with strong time management and organizational skills.
- Proactive, empathetic, and experienced in engaging individuals experiencing homelessness.
- Related work experience in public health, health outreach and education
- Fluency in another language such as Spanish, Russian or Korean preferred.
