Job Description

The Medicare Mental Health Kids Hub is an early intervention service designed to support children aged 0–12 years and their families and caregivers. Through a dedicated multidisciplinary team, the Hub will deliver a broad range of evidence-informed supports, including therapeutic interventions, group programs, parenting support, peer and family support, cognitive assessments, and outreach services, tailored to meet the needs of children and families. Part-time & Full-time position available.

The Role:

The Mental Health Outreach Worker delivers recovery-oriented, trauma-informed support to children and their families/carers using a case management approach. Working primarily in community settings, the role requires the ability to effectively manage a diverse caseload, demonstrating strong organisational, time management and administrative skills.

Responsibilities:

  • Provide case coordination, case managment and therapeutic intervention.
  • Conduct standaridised assessments, intake, coordination reviews and case management.
  • Maintain accurate case notes.
  • Work across multidisciplinary teams, including partners, stakeholders and interagency members to achieve the best possible outcomes.
  • Develop and implement psychoeducation to benefit young people targeting skill development, building resilience and protective factors.

Qualifications:

  • Tertiary qualification in Psychology, Social Work, Counselling or related field for membership of AASW or registration with AHPRA or with ACA or PACFA (level 3 or 4).
  • At least 3 years demonstrated experience working with young people and/or families with complex needs.
  • Demonstrated verbal and written communication skills, organisational time management skills.
  • Ability to respond appropriately to crisis situations and to clients presenting with complex needs.
