Job Description

JOB TITLE: Intensive In-Home Specialist

SCHEDULE: Full Time

LOCATION: Seeking in Multiple Locations!

Wake County/Cumberland County/Guilford County

REQUIRED EDUCATIONAL/EXPERIENTIAL QUALIFICATIONS:

  • Completion of Bachelors Degree, within Human Service field preferred.
  • Meets requirements under 10A NCAC 27G.0104 as an Associate or Qualified Professional for Mental Health.

POSITION HIGHLIGHTS:

  • Provide direct services through community agencies and in-home services to support family preservation.
  • Deliver one-on-one interventions such as skill building activities, intervene with positive behavioral supports, enhance consumer’s knowledge of interpersonal and community relational skills.
  • Willing to deliver services in a wide range of community settings such as detention centers, schools, homes, jails, libraries, homeless shelters, and any other applicable locations deemed appropriate and safe.
  • Be available as part of a three-person team to ensure 24/7/365 crisis response. Face-to-face crisis management must be available in no less than two hours.
  • Perform ongoing assessment and evaluation of the social and daily living needs of clients to ensure seamless transition from this enhanced service to the next medically necessary service.
  • Provides supportive and informational counseling to include crisis intervention, case management, and the development of collateral contacts.
  • To have a network of referral agency contacts and resources in order to fulfill the socioeconomic needs of the clients.
  • To assist clients with housing, employment, social, legal, transportation and educational needs, by connecting them with community services.
  • Completes NCTOPPS with consumer if applicable.

ADDITIONAL INFORMATION:

  • Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health.
  • Considerable knowledge of mental health and substance abuse disorders.
  • Considerable knowledge of available community resources, services and the coordinating efforts needed for clients with mental health and substance abuse problems.
  • Considerable knowledge of treatment modalities for mental health disorders.
  • Some knowledge of the principles and practices of substance abuse treatment and education activities.
  • Ability to respectfully represent the agency and establish a rapport with the community.
  • Ability to determine appropriate community services for clients and interface community resources and agencies to meet client needs.
  • Ability to communicate effectively in oral and written forms.
  • Ability to network and coordinate efforts between outreach, treatment, and other support systems for the clients.
  • Ability to work well on a team and coordinate efforts to meet the needs of the client.
  • Must be able to physically perform the basic life operational functions of climbing, stooping, crouching, reaching, pushing, pulling, lifting, standing, walking, fingering, grasping, talking, and hearing.
  • Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift objects.
  • Must possess the visual acuity to perform extensive reading and determine the accuracy of work assigned.
  • Must have a valid driver’s license and be able to work in community settings.
