Job Description

Status: Full Time Hourly FLSA classification: Non-Exempt

of the Position The Intensive Family Coordinator will provide a professional level of case management services as well as direct advocacy services to a caseload of families.

  • The Intensive Family Coordinator will be responsible for completing family assessments and creating individualized service plans.
  • The Intensive Family Coordinator will be responsible for providing services for truancy program in the school district. The Intensive Family Coordinator will report to the Director of the Program. Successful applicants will possess outstanding people skills, creativity, and energy as well as expertise of Essex County and its resources.
  • Applicants will be versed in truancy, school issues, and assessing family dynamics and issues. YAP, Inc. relies heavily on the use of informal supports drawn from resources found in the home neighborhoods of client families to build a network of support for each youth and family referred.
  • Travel will be necessary as well as willingness to work nights and weekends and provide 24/7 crisis support to the caseload.

Pay $45,000-47,840

Qualifications/Requirements:

  • Minimum High School Diploma or GED Equivalent.
  • Experience working with at-risk youth and families
  • Knowledge of community resources
  • Proficient in computer skills and Microsoft Office: experience using an Electronic Health Record (EHR) System is a plus.
  • Position requires reliable transportation, valid driver's license, and current auto insurance coverage
  • Bilingual (Spanish Speaking) is a plus.
  • May require evenings and weekends
