Job Description

Status: Full Time Hourly FLSA Classification: Non-Exempt

of the Position: Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families.

  • Intensive Family Coordinator will be responsible for completing family assessments, recruiting foster parents, completing home studies for the referring authority, locating family resources, and creating individualized service plans.
  • Case management services including intakes, safety planning, team meetings, ISP development 24/7 support, case contacts, and case record documentation in keeping with state regulations.

Qualifications/Requirements:

  • High School Diploma or GED is required.
  • Experience with community work and at least one (1) year experience working in human services field.
  • Experience in the provision of home and community based services.
  • Proficient in Microsoft Office Suite cand computer skills. Experience using an electronic health record (EHR) system is a plus.
  • Strong communication and organizational skills
  • Must have reliable transportation, valid driver's license, and current auto insurance coverage.
  • Bilingual (Spanish Speaking) is a plus.
  • May require evenings and weekends
