Adult Intensive Case Manager Summary

This full-time role manages a caseload of approximately 15–20 adults with severe and persistent mental illness who require a higher level of support than standard case management. Reporting to the Assistant Program Manager, Intensive Adult Case Management, the position provides community-based psychiatric support and treatment interventions for clients age 18 and older with complex, multi-service needs. Key responsibilities include developing collaborative treatment plans, supporting clients in achieving their goals, and delivering services in community settings. The role requires strong time management, the ability to work independently, and effective collaboration within a multidisciplinary team.

The Adult Intensive Case Manager works under the direct reporting of the Assistant Managers of

Traditional Case Management and in guidance / collaboration with other PHS Leaders.

A Day in the Life

• Conduct ongoing assessments of clients and families, monitoring symptoms and supporting self-management to promote optimal functioning.

• Provide crisis intervention and support for clients experiencing acute distress.

• Educate and connect clients and families to community resources; facilitate access to needed services.

• Coordinate and monitor service delivery; advocate on behalf of clients across providers.

• Provide education on mental health, symptoms, and medication.

• Promote recovery, self-sufficiency, and development of natural supports.

• Strengthen coping and interpersonal skills through individualized, community-based interventions. Meet productivity standards and performance benchmarks.

• Complete documentation, reports, and clinical records accurately and within required timelines; maintain compliance with agency standards.

• Administer required assessments and surveys as scheduled.

• Complete incident reports as needed and maintain proficiency with agency systems.

• Attend and actively participate in required meetings, case conferences, and treatment team sessions.

Requirements

Education/Experience

Experience providing case management to adults with severe and persistent mental illness and dualdiagnosed populations preferred. A Bachelor’s degree, Associate’s degree, or High School Diploma

with mental health related experience.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance

instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to

speak effectively before groups of customers or employees of organization.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where

only limited standardization exists. The ability to interpret a variety of instructions furnished in

written, oral, diagram, or schedule form.

Computer Skills

To perform this job successfully, an individual should have the ability to gain knowledge of current

practice management system, electronic medical record, Microsoft Word, text paging, Internet, and

Intranet.

Other Applicable Requirements

Excellent communications skills, both written and verbal. Ability to work effectively and

independently with all levels of clinical and administrative staff within the health centers and with

community leaders. Strong leadership qualities and leadership experience. Demonstrates program

development and implementation skills. Ability to represent the organization effectively in a variety

of settings and with diverse communities. Demonstrates understanding and appreciation for diverse

cultures.
