Qualifications:

• High School diploma or equivilent
• Three years of responsible full time clerical experience
• Excellent customer service skills
• Ability to follow complex oral and written instructions

Duties and Responsbilities:

• Assist individuals served by addressing inquiries, resolving issues, and fulfilling requests across diverse communication channels.
• Responsible for answering and triaging internal and external telephone calls. Conducts initial screening of individuals serviced and other patrons to determine the nature of requests or problems.
• Pulling, filing, auditing, and maintaining charts for the teams assigned.
• Closing team charts
• Co-facilitating team meeting minutes including huddles regarding team-based care, integrated health and other population health.
• Reviewing and sharing data reports, outcomes of team services.
• Assiting with coordination of care for individuals served on the team.
