Who We're looking for

Copa Health is seeking a Full-Time passionate Integrated Care Coordinator that will function as a core member of an integrated care team that involves the member’s primary care provider, psychiatrist, case coordinators, care managers, other healthcare providers, and ancillary staff, with a focus on management of individuals identified on the High-Risk Registry, and individuals with more complex medical and psychiatric conditions who are not progressing as expected.

Key Responsibilities
  • Assists the clinical team and population health team in meeting Value-Based initiatives utilizing innovative techniques to increase Clinical Team performance and member concordance with these measures.
  • The ICC will work with the Clinic Leadership to identify performance outliers by Clinical Team including assistance with tracking and monitoring gaps in care and clinical outcomes using available population health tools.
  • The ICC will work alongside the clinical and physical health teams to provide direct member care and engagement as needed to meet the Value Based measures and outcomes.
  • The ICC may carry a short-term caseload of individuals who are frequent utilizers of inpatient and emergency department settings, and others with more complicated care needs as determined by the site leadership, Population Health Department, treating BHMP and/or PCP.
  • Coordinates with the Copa Health Care Management Staff in identifying potential causes of high utilization and assists the Integrated Care Team in designing and modifying interventions to address the issues identified.
  • Participates in morning meetings, daily huddles, and all staff meetings.
  • Documents in person and telephone encounters in the EMR and uses the system to identify and re- engage members who may be lost to followup.

Qualifications

  • Requires a Bachelor’s degree in the behavioral health field plus one (1) year of full-time direct care experience. OR
  • High school diploma or high school equivalency diploma and five (5) years of full-time direct care experience in behavioral health.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
  • TB Test clearance required at the time of hire.
  • bilingual (English/Spanish) preferred.
