Schedule: Monday–Friday, 8:30 a.m. – 5:00 p.m.

Position Summary
Hill Country Community Clinic is a Federally Qualified Health Center (FQHC) and Certified Community Behavioral Health Clinic (CCBHC) providing integrated, whole-person care to individuals and families with complex behavioral health and medical needs.
The Integrated Care Coordinator supports access to Behavioral Health, Case Management, and Substance Use Disorder (SUD) services by coordinating intake, referrals, scheduling, and follow-up. This position works closely with multidisciplinary care teams to ensure timely, accurate, and compassionate support for patients navigating behavioral health services.

Essential Duties and Responsibilities
- Coordinate intake, scheduling, and follow-up for Behavioral Health, Case Management, and SUD referrals
- Receive, review, and process CCBHC referral documentation, releases of information (ROIs), and consents
- Conduct outreach to prospective CCBHC clients to schedule intake appointments and document all attempts in the EHR
- Update and maintain accurate patient demographics, insurance, and enrollment information
- Verify insurance eligibility and behavioral health benefits
- Provide professional and empathetic support to individuals experiencing mental health or substance use challenges
- Receive warm hand-offs from providers and assist patients with basic navigation and service needs
- Answer phones, schedule appointments, and support front-office and CARE Center intake functions as needed
- Collaborate with providers, case managers, and care coordination teams
- Participate in team meetings and program-related activities
- Perform other duties as assigned
Minimum

Qualifications
- High school diploma or equivalent required
- Some college coursework and at least one year of related experience, or an equivalent combination of education and experience
- Intermediate computer skills, including Microsoft Office and electronic health record systems
Preferred

Qualifications
- Experience in a medical, behavioral health, or community health setting
- Familiarity with HIPAA and behavioral health confidentiality requirements
- CPR certification

- Ability to work effectively with individuals experiencing emotional distress or behavioral health challenges
- Ability to multitask, prioritize, and adapt in a fast-paced clinical environment
- Attention to detail and strong documentation skills

Work Environment and Physical Requirements
This position works in an indoor clinical/office environment with moderate noise levels. Duties require regular sitting, use of hands and arms, talking and hearing, and occasional standing or walking. Ability to lift up to 25 pounds with assistance may be required.
CCBHC-Specific Program Disclaimer
This position is part of Hill Country Community Clinic’s Certified Community Behavioral Health Clinic (CCBHC) program and is supported by federal grant funding. Job duties, reporting requirements, performance expectations, and program priorities are aligned with CCBHC federal and state standards and may change based on regulatory guidance, program evaluation, or funding requirements.
Continued employment in this position is contingent upon ongoing CCBHC funding and program authorization. While Hill Country Community Clinic intends to apply for continued funding, continued funding is not guaranteed.
Current grant funding for this position is scheduled to end September 30, 2026.
