Partnerships with local businesses like Stew Leonard, TJ Maxx, and Marshalls have allowed the vision to grow exponentially. In addition, taking on residential programs has allowed us to serve those that need round-the-clock supervision.

Job Title: IntakeOutreach Specialist

Reports to: Director of Intake and Social Services

Location: Bronx, NY

FLSA: Non-exempt

Job Classification Full-time

Hours/Days: 40 hours per week

Shift: Monday-Friday & some weekends

Salary Range $25 -$27 per hour

The Intake Outreach Specialist is responsible for promoting the agency's programs and services, developing relationships with referral sources, and increasing awareness of available support for individuals with intellectual and developmental disabilities. This position serves as the primary liaison between the agency and Care Coordination Organizations (CCOs), schools, community organizations, Hospitals, families, discharge planners, and other referral partners. The Intake Outreach Specialist works closely with the Intake Specialist to facilitate referrals and support individuals and families throughout the enrollment process.

Essential Duties & Responsibilities:

• Develop and maintain positive relationships with OPWDD stakeholders, including Care Managers, CCOs, hospitals, school districts, social workers, discharge planners, and community organizations. ·
• Promote agency programs, including Residential Services, Community Habilitation, Day Habilitation, Employment Services, and any other programs.
• Conduct outreach visits, presentations, and informational meetings to educate referral sources about agency services.
• Attend community events, resource fairs, provider meetings, and networking opportunities to increase agency visibility.
• Respond promptly to referral inquiries from individuals, families, and professionals.
• Collaborate with the Intake Specialist to ensure timely processing of referrals and admissions.
• Assist prospective individuals and families in understanding OPWDD services, eligibility requirements, and the enrollment process.
• Maintain regular communication with referral sources regarding service availability and program updates.
• Prepare weekly/monthly outreach reports detailing referral trends, marketing activities, and enrollment outcomes.
• Take inventory of marketing items & place replenishment orders.
• Work collaboratively with Intake Specialist to identify service vacancies and coordinate admissions.
• Ensure all outreach activities are conducted in accordance with OPWDD regulations, agency policies, and confidentiality requirements.
• Assist with therapeutic and CCO referral.
• Represent the agency professionally and maintain a positive public image.

Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, Public Health, or a related field preferred.
• Minimum of two years of experience working with individuals with intellectual and developmental disabilities, healthcare, human services, or community outreach preferred.
• Bilingual (Spanish)
• Knowledge of OPWDD services, eligibility requirements, HCBS Waiver services, Care Coordination, and person-centered planning is strongly preferred.
• Proficiency with Microsoft Office and electronic record systems.
• Some experience in marketing is preferred.
• Valid driver's license and reliable transportation for community outreach activities. (Must be willing to drive to work in Queens, Westchester, Bronx, at times some parts of the Hudson Valley Region.
