Be the Best Admissions Manager You Can Be

If you meet these qualifications, we want to meet you!

  • Associates Degree, Preferred
  • Hospice, home health, or case management experience
  • 1+ years of supervisory experience or AccentCare tenure
  • Excellent written and verbal communication skills
  • Ability to work closely with a wide variety of stakeholders

Responsibilities:

  • Ensure all patient referrals are received in a timely and complete manner
  • Coordinate with staff to ensure responsibility for the initial determination of a patient and their families service needs
  • Supervise communication with providers, attending physicians, other appropriate hospice staff members, and any
  • other agencies as needed to coordinate initiation of hospice services
  • Maintain regular communication with the Director-Business Development (DBD)
  • Assist callers with information about other community resources when hospice is not appropriate
  • Maintain referral statistics and provide reports on a monthly basis to DBD
  • Build and maintain close referral and third-party payor relationships through appropriate and timely written and phone communication
  • Maintain confidentiality of sensitive information
  • Coordinate with Admissions Department, personnel assignments to cover admissions needs and assessments in an effective and timely manner
  • Work cross-functionally with Director-Clinical Operations (DCO), Team Directors (TD) and DBD to ensure promised services are provided
  • Participate in hospice orientation and ongoing education programs
  • Participate on committees and in special projects, as assigned, including those related to fiscal responsibilities
  • Ensure all data is accurately entered into computer systems within guidelines
  • Actively participate in short and long-term planning
  • Initiate creative ways to meet the needs of patients and families
  • Serve as point of contact for Hospice Nurses with admissions responsibilities – including clinical skills oversight, assignments and monitoring compliance regulations
  • Completes and provides 90-day and annual performance reviews for direct reports
  • Monitors compliance of all regulatory requirements
  • Provides follow-up and resolutions for both internal and external concerns and complaints
  • Other duties as assigned
