Intake Coordinator/RN - Behavioral Health Services, West Plains, MO

Signet, in partnership with Ozarks Healthcare, is seeking an experienced behavioral health professional to serve as an Intake Nurse Coordinator/ RN for our comprehensive continuum of behavioral health services.

Signet Health offers a comprehensive benefits and compensation package, with a budgeted salary range of $70K to $83K based on experience.

The Intake Coordinator facilitates timely access to care by managing referrals, gathering and reviewing clinical information, evaluating patient needs, and coordinating placement into the most appropriate level of care. This position serves as a central point of coordination and helps patients navigate behavioral health services across the continuum, including crisis services, inpatient care, and outpatient behavioral health services.

Key Responsibilities:

  • Intake Answering inquiries in an expeditious manner, assessing appropriateness of admission based on criteria, determining appropriate level of care, and overall management of the initial intake process.
  • Clinical Coordination: Reviewing referrals for appropriate levels of care and coordinating with clinical staff for case assignment. Attend treatment team meetings and facilitate movement of patients through the various levels of care as appropriate which could be at various locations where services are provided.
  • Support of special needs for patients: Assist with obtaining specialty supplies needed for patients.
  • Crisis Management: Handling urgent calls and connecting patients to appropriate emergency services if needed.
  • Communication: Acting as a liaison between patients, providers, and external referral sources. Excellent customer service skills for both internal and external customers.
  • Follow up: Follow up with patients who do not present to their next level of care as scheduled.
  • Respond promptly and professionally to patients, providers, and referral-source inquiries.
  • Manage the initial intake and referral process across the Ozarks Healthcare behavioral health continuum.
  • Gather and review clinical, psychosocial, and referral information to determine whether services are appropriate and identify the recommended level of care.
  • Coordinate with providers, nursing, clinical staff, and program leadership to support timely evaluation, acceptance, placement, and case assignment.
  • Serve as a navigator for patients and families by explaining available services, assisting with transitions, and connecting patients to the appropriate program or location.
  • Participate in treatment team and care coordination meetings, as appropriate, to support patient movement between levels of care.
  • Facilitate transitions among crisis, inpatient, and outpatient services based on the patient’s clinical needs.
  • Monitor referrals and follow up with patients who do not present for their scheduled next level of care.
  • Communicate with internal departments and external referral sources to promote continuity of care and reduce barriers to treatment.
  • Handle urgent or crisis-related calls and connect individuals with the appropriate crisis or emergency services.
  • Assist with coordinating specialty supplies or accommodations needed to safely support patients receiving services.
  • Maintain accurate, timely, and complete documentation in the electronic health record and other required systems.
  • Provide excellent customer service to patients, families, providers, community partners, and internal team members.
  • Protect patient confidentiality and ensure compliance with HIPAA, organizational policies, and applicable regulatory requirements.
  • Perform other duties as assigned to support access, coordination, and continuity of behavioral healthcare.

Requirements/Qualifications

Qualifications:

  • Missouri RN license required; BSN is preferred
  • 5 years practicing in a behavioral health setting with at least two of those years on an inpatient unit required
  • Certified in Psychiatric Nursing preferred
  • Thorough understanding of Trauma-Informed Care
  • Strong communication, interpersonal, organizational, and computer skills (EHR systems, Microsoft Office).
  • Ability to handle stress, maintain confidentiality (HIPAA), and demonstrate empathy for clients.
  • Travel to various behavioral health sites required
