Job Description

Sun River Health provides the highest quality comprehensive primary, preventative and behavioral health services to all who see it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Full-time Intake Coordinator of Adult Health Care to join our team in Inwood Bronx, NY.

The Intake Coordinator of Adult Health Care is assigned to streamline intakes and assessments while working closely with the medical and completing all admissions paperwork and welcoming clients to the facility.

Responsibilities:

  • Acquire necessary documents within the specified time frame to convert referrals into admissions
  • Adhere to performance standards based on program specifications and level of service requirements, including completion of all paperwork in a professional mannerwithin the designated time frames.
  • Verify Medicaid eligibility for prospective clients.
  • Demonstrate professional responsibility in making effective use of individual and group supervisory sessions, program staff meetings, agency resources and training sessions.
  • Participate in available training provided both in-service and through outside resources. In addition, utilize agency films, books and literature.
  • Maintain regular contacts with referral sources to recruit program applicants.
  • Maintains program pending list for non-compliant former applicants; submits monthly reports to supervisor regarding pending list status and intake activities.
  • Schedules and process applicants’ entry into the program; provides pertinent information to assigned case manager and other program staff during morning rounds.
  • Research and educate staff on available community resources
  • Conduct orientation groups for newly enrolled clients.

Pay: $24.00 - $27.35 per hour

In the early 1970s, four African American women in Peekskill, New York came together to address the lack of affordable, quality health care in their community. With money from a small federal grant, the first Sun River Health site was opened in 1975.

That first health center has since grown to become an FQHC (federally qualified health center) system with over 40 locations, serving more than 250,000 patients throughout the Hudson Valley, New York City, and Long Island. Our exceptional primary care practitioners, specialists, and support staff have made us a destination for affordable, high-quality care.

In December 2018, Brightpoint Health, an FQHC network offering integrated medical, behavioral, and social support services in New York City, merged with Hudson River Health Care. Together with our subsidiary organizations, we became Sun River Health in December 2020.
