The Clinical Intake Coordinator, LPN facilitates the Hospice referral process by receiving referrals from multiple referral sources while collaborating with Hospice Care Consultant. Determines need for additional information from referral sources. Communicates agency acceptance or decline of referral to varied referral sources, schedules admissions RN, and processes accepted referrals, and audits admission consents.
Prioritize and triage hospice referrals. Screening for clinical eligibility, DME and Payor information, and Ensures regulatory compliance is met during initial screening and intake process. Employees are required to work on-site 5 days per week.
