The Intake Coordinator-Home Infusion facilitates the referral and intake of patients for home infusion services from a variety of settings, including acute care, post-acute facilities, physician practices, payors, and patients/families. Provides screening, assessment, planning, and coordination of services to support timely initiation of home infusion therapy. Maintains working knowledge of federal and state regulations, accreditation standards, and medical and pharmacy payor requirements governing home infusion services. Utilizes strong communication and customer service skills to ensure effective coordination among referral sources, pharmacists, nurses, and operational teams. Works collaboratively with the referral and intake team to ensure patients receive the right therapy at the right time in the appropriate care setting.

Collects, reviews, and processes referral and patient information accurately and efficiently from multiple sources (e.g., providers, case managers, electronic health records, referral portals) to support timely initiation of home infusion services.
Coordinates intake and onboarding activities for referred patients, including verification of referral completeness, identification of therapy requirements, and screening for clinical, payer, and operational considerations that may impact home infusion services.
Communicates patient specific clinical and demographic information necessary for development of the infusion plan of care to pharmacists, nurses, and other interdisciplinary team members, escalating complex or urgent referrals to supervisory personnel as appropriate.
Reviews all incoming referrals for completeness and appropriateness for home infusion services, including assessment of medical necessity, therapy suitability, and alignment with organizational guidelines and payer requirements..
Collaborates with the referral and operations teams to identify scheduling needs for nursing visits, medication compounding, delivery, and start of care timing for each accepted referral.
Screens patients for eligibility for home infusion services based on established clinical criteria, payer coverage, and operational capabilities, ensuring referrals are accepted or declined appropriately and consistently.
Maintains accurate intake, referral, and admission data, tracking key metrics related to referral volume, acceptance/denial trends, and time to service initiation; prepares reports for leadership as requested..
Participates in performance improvement and quality initiatives related to intake operations, contributing data and feedback to support optimization of workflows, productivity, and patient access to services.
Provides cross coverage and support for other intake or administrative functions as needed based on staffing and operational demands, including data entry, scanning, faxing, filing, and document management.
Performs other duties as assigned.

This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - High school diploma or equivalent
Experience - No experience is required. Two (2) years home infusion, specialty pharmacy, and/or medical intake/reimbursement experience preferred.
