Job Description

The Salvation Army Midtown Services and Treatment Center is looking for a Intake Coordinator. This individual will be responsible for counseling, conducting intakes, assessments, crisis intervention, developing treatment plans, ongoing follow up counseling and treatment plan reviews, developing safety plans, group therapy and education, and facilitating referrals.

This is a full-time position, 40 hours per week with starting pay of $15-17 per hour depending on experiences.

Essential Functions

1. Conduct intake/admission including collecting basic demographic and background information, comprehensive needs, abilities, strengths and preferences, drug use history, Standard Means Test and orientation to MSTC, services and expectations. Utilizing this information to assigning guest to appropriate level of care and program.

2. Respond to new referrals documenting in appropriate method.

3. Learn and become familiar with all aspect of intake and program assignment functions in CIMOR and with the EHR software and collection system.

4. Correct any file/charting deficiencies within appropriate time frame.

5. Work with team to provide Screening on phone or face-to-face.

6. Offer administrative support to clinical staff in completion of letters, forms, referrals etc.

7.Enter data into computer data base within the appropriate timeframes (EMR, CIMOR).

8.Coordinate with billing department to assure all admissions/transfers/discharges are completed timely

9. Enter and assign guest to Bed Board and residential roster

10.Update class and activity participation log

11.Maintain, update, and submit waiting list according to CARF and DMH standards.

12.Attend staff meetings, departmental meetings, seminars/conferences, and in-service training programs.

13. Maintain confidentiality and comply with Safe From Harm guidelines.

14. Maintain positive, professional relationships with referral sources, clients, coworkers, and leadership adhering to Code of Ethics and TSA MSTC Policies

Minimum Qualifications

High school diploma or equivalent and a minimum of two (2) years’ related experience, required Proficient experience with online billing software and databases (CIMOR experience is a plus), Microsoft Office Suite and use of E-mail, requiredPosition requires above average communication skills with clients and staffCPR/First Aid certification, requiredAnnual TB test, requiredMust be a team player, a self-starter, and perform well with minimal supervisionMust think independently and demonstrate sound judgment.

Physical and Working Conditions

Must be capable of performing the above essential duties in an office environment to include using a computer, answering phones, moving client files, etc Ability to lift to 20 pounds, frequently and up to 40 pounds, occasionally. Most work will be indoors in a temperature-controlled environment.
