Your Role in Our Mission

As an Intake Coordinator, you play a key role in ensuring individuals experience a smooth, supportive, and timely entry into services. Your work will focus on coordinating and overseeing the intake process, from initial screening to successful referral and follow-up.

You will engage with individuals in a variety of settings — including homes, hospitals, shelters, and community locations — and collaborate closely with internal programs and external partners to ensure individuals are connected to the appropriate level of care.

Key Responsibilities Include:

• Conduct pre-assessment screenings (in person or by phone) and schedule comprehensive clinical assessments to determine appropriate levels of care
• Complete intake, opening paperwork, and required documentation accurately and in a timely manner
• Develop and provide crisis plans, including Crisis Response contact information, and assist with crisis response as needed
• Coordinate and link individuals to appropriate internal and external services, following up to ensure treatment initiation
• Maintain current knowledge of community resources and serve as a liaison among individuals, families, staff, providers, and community partners
• Provide high-quality customer service while representing the agency professionally and compassionately

Compensation & Benefits

We offer a competitive compensation package ($42,000-$50,000 annually DOE) and comprehensive benefits for benefits-eligible positions, including:

What We’re Looking For

Minimum Qualifications:

• Bachelor’s degree from an accredited institution (human services preferred)
• Mental Health, Substance Use and/or IDD experience strongly preferred
• Experience providing case management services with knowledge of DSM-V diagnoses, service options, and community resources
• Strong communication skills with the ability to work effectively in a fast-paced environment
• Demonstrated ethical standards with no substantiated findings on the NC Healthcare Registry
• Valid driver’s license, current auto insurance, and ability to travel as required
• Ability to work a flexible schedule (including evenings/weekends)
